Abstract

Non-communicable diseases (NCDs) such as Diabetes Mellitus (DM) and Gastroesophageal Reflux Disease (GERD) have become growing public health challenges, including in the community of Jl. Nagur Gg. Manunggal, Pematang Siantar City. Lack of public knowledge and awareness regarding the risk factors, symptoms, and prevention efforts for these diseases often leads to delayed treatment and increased risk of complications. This community service activity aimed to improve family health literacy through nursing-based health education on the prevention and management of DM and GERD. The method used was an educational approach, including direct health counseling, leaflet distribution, and interactive discussion sessions. The results showed increased community understanding of the importance of healthy lifestyle habits, dietary control, and the role of the family in supporting members at risk or suffering from chronic diseases. This activity is expected to empower families as the frontline unit in preventing and managing chronic conditions independently within the household.
